Baseball 4 Life ELITE
Application Form
Players wanting to participate in the Baseball 4 Life ELITE program must qualify to work with us. Those qualifications include baseball or softball ability, the desire to learn, a strong work ethic, and a good fit with the other players that are already in the group.
Whether a player qualifies for this program or not is decided by the Diamond Dreams staff. It may require a personal interview and/or lesson to evaluate the student both as a player and as a person.
Also, once a player has been accepted to the ELITE program they must continue to earn the right to be there. If a staff member deems that a player is disruptive to the group or fails to continually meet the qualifications that got them into the group they will be suspended for a week or asked to leave the class. In either case a refund will NOT be given. By filling out and signing this application form both the parents/guardians and the player are agreeing to this policy.
